Financial Considerations
There are many financial
considerations to think about when moving into the community.
How will you pay the bills?
What will the bills be?
What services are you going to need to be successful in your move?
A big thing that you'll have to figure out is how you are going
to get furniture, dishes, pots and pans, and all of the household
things that you will need in your own home. You can find
some of these things in thrift stores or from friends and
relatives. Make a list of things you need and where you'll
get them.
Nursing Home
Transition Grant
When you are getting ready to move from
a nursing home you need to be screened for a CADI waiver (which
can be done while you are still in the nursing home) You may
be able to access a one time grant of up to $3,000 for your
transition costs
There are limits on what the
$3,000 can be spent on:
$1,000 furniture
$300 household supplies
$609.60 transitional services
$1,090.40 deposits & window coverings
The county must preauthorize
the grant, and the grant DOES NOT affect the other CADI waiver
services such as home access, etc...
